Lokahi Treatment Centers (LTC) is a nonprofit that operates six treatment facilities across Hawaii. The facilities are located in Hilo, Honoka’a, Kailua Kona, Kohala, Pahoa, and Waikoloa. This review focuses on the Hilo facility is located on the Big Island, and provides outpatient-based treatment programs for adults and adolescents struggling with chemical dependency and co-occurring mental health issues.

The Hilo location also offers anger management and domestic violence intervention (DVI) programs. In addition to its outpatient programs, LTC of Hilo recently began operating Lokahi House, an eight-bed sober living facility for men.


The center’s website notes before participating in treatment, clients undergo an initial substance abuse and psychiatric assessment. If applicable, LTC also conducts anger management and DVI assessments. In Hawaiian, “lokahi” means harmony and balance, and according to the program’s website, Lokahi Treatment Centers’ goal is to provide treatment “centered on personal responsibility, individual dignity, and self-esteem.”

To achieve this goal, substance abuse outpatient programs involve group counseling sessions that identify client strengths and address underlying issues that contribute to addictive behavior. Adolescent group sessions are co-ed, while adult sessions are gender-specific. Group sessions are held in the mornings, afternoons, and evenings to accommodate client work or school schedules. Individual therapy and family counseling sessions are also incorporated into treatment. In addition to counseling, clients with a dual diagnosis receive ongoing medication management to alleviate the symptoms of mental health issues.

To reside in the Lokahi House, clients must be enrolled into LTC’s outpatient program and meet educational, job training, or employment milestones. Individuals involved in the criminal justice system (probation, parole, reentry) are welcomed. Upon completion of the outpatient program, all clients receive referrals to community-based resources and agencies for continued recovery.


CARF-accredited, Lokahi Treatment Centers of Hilo employs a team of accredited substance abuse counselors and a program medical director, according to the center’s website. Lokahi House is supervised by an on-site manager, and the organization offers internships and volunteer opportunities.


Able to accommodate up to eight male residents, Lokahi House provides a stable, sober environment conducive to recovery, the center’s website indicates. Residents have access to job training and employment opportunities, and transportation is provided to treatment sessions, court, and other necessary appointments. No information is provided concerning the average length of stay.


To date, the one alum who was polled by Rehabs.com had a positive experience at the Lokahi Treatment Center in Hilo. They gave the center four out of five stars when asked if they would recommend the center, and five stars for treatment effectiveness.

When asked what were the center’s strengths, the alum wrote “everything.”


The two loved ones surveyed by Rehabs.com provided mixed opinions of the program. One loved one gave the center five out of five stars when asked if they would recommend the Hilo facility.

They also gave the center five stars when asked about the staff’s experience and training. “They employ Counselors with experience in the field,” they wrote. “They have a clinical supervisor that spends time with his employees.”

However, the other loved one gave the center two stars when asked if they would recommend it, and rated the center’s counseling options the worst with one star. “Lokahi drug and alcohol rehabilitation out patient services to no segreate different types of addiction,” they wrote.


According to the facility’s website, Lokahi Treatment Centers accepts most medical insurance plans.

There are some helpful counseling sessions. Other participants are there because of mandatory prison orders and are a threat to the health and safety of others. Lokahi drug and alcohol rehabilitation out patient services do not segregate different types of addiction. Participants are encouraged to talk any way they want to other participants. The sessions concerning social events like pot luck are not mandatory and therefore those suffering from social interaction problems will not have those counseling sessions.

This program works with the clients referral and Drug Court program. Our town is small and you can count all facilities on one hand. Strengths: They employ Counselors with experience in the field. They have a clinical supervisor that spends time with his employees. There are incentives available to the employees. Besides substance/alcohol abuse they provide Co-occurring and DVI classes. Weaknesses: Time between a relapse discharge to continued care.
