Founded in 1975, Crossroads Behavioral Health Services (CBHS) operates seven centers in southwestern Iowa, offering outpatient-based treatment for individuals struggling with substance use, mental health conditions, and co-occurring disorders. The organization's locations are among the 127 private, nonprofit substance abuse treatment facilities located in Iowa.


To treat individuals struggling with substance use, CBHS provides an intensive outpatient program (IOP), general outpatient treatment, and an extended care program. Of note, in Iowa, outpatient services comprise 90.2 percent of the type of care available to help those with substance use disorders.

At CBHS, counseling occurs in individual and group settings, and the frequency of treatment is determined by the client’s needs and program intensity. Counselors use evidence-based practices, such as motivational interviewing and dialectical behavior therapy (DBT), to help clients set goals and improve coping skills.
CBHS also offers a Women’s Grant Program, which offers specialized programming for mothers and pregnant women struggling with substance use disorders. The service provides childcare while the mother attends therapy, as well as parenting skills training, transportation reimbursement for sessions, and coordinates doctor appointments for both mother and child.

Psychiatric services including medication management, counseling sessions, educational classes, and extensive mental and behavioral health services for families and couples.


The CBHS team includes psychiatrists, nurses, and licensed social workers who specialize in substance abuse and mental health counseling. Of the three individuals polled by to date, two survey respondents gave perfect five-star ratings regarding the staff’s level of training and experience, while the third individual gave this aspect of the facility two stars.


The two loved ones polled by to date offered polarizing feedback for the facility. One individual offered perfect five-stars ratings for a majority of survey metrics, including the level of family participation, its discharge planning, counseling options, and ability to treat co-occurring disorders. Regarding the Creston location, the loved one also appreciated the facility's central location. However, the other loved one gave two stars for most metrics, including the level of family participation and the facility's counseling options. They also described staff as "uppity."


The sole staff member polled by to date indicated that they would recommend CBHS to friends and family. The staff member gave perfect five-star ratings for the credentials of the mental health staff, the staff's willingness to put the client's interest first, and the medical staff, though did report recent changes to medical staff.


CBHS accepts Medicare, Medicaid, private insurance, and self-pay. It also offers a sliding fee scale for qualifying clients, and provides treatment via the federally-funded Access To Recovery (ATR) program, which is available to Iowa residents who are “at or below 200% of the federal poverty level guidelines.”

Location was nice. Parking was not great. This facility is in a great location as far as centrally located however parking is lacking.
