First Step Services LLC Philosophy

The primary goal of First Step Services, LLC is to assist adolescent and adult clients to end all problems they have with use of alcohol and other drugs. No matter how small or large, we can assist in putting them behind you.

First Step is a NC Department of Health and Human Services licensed substance abuse and mental health facility, serving persons who need assistance with a wide range of substance abuse and mental health needs. Our mission is one of quality care, education and research for each patient and their families and significant others.

Areas of Specialization

  • Individualized Treatment
    Some facilities have an addiction treatment protocol that all patients or clients are expected to follow, while others customize or individualize treatment based on a person’s unique needs and circumstances. Factors that may affect treatment decisions include age, lifestyle, medical conditions, type of drug, religious beliefs, etc.

  • Intensive Outpatient (IOP)
    Patients who undergo intensive outpatient treatment continue to live at home and sometimes go to school or work while participating in a highly structured treatment protocol that is focused on ending substance abuse. Programs vary in terms of how much treatment patients receive, how often and for how long. Some facilities design individualized intensive outpatient treatment programs.

First Step Services, LLC operates three treatment facilities in North Carolina. This review focuses on the center in Raleigh, which offers substance abuse outpatient treatment, intensive outpatient programs, along with individual and family counseling. Detox is not available on-site.


First Step Services, LLC is one of North Carolina’s 194 (41.1 percent) facilities with an IOP program. It focuses on getting clients into counseling and helping them change their lives.

First Step's intensive outpatient program (IOP) typically lasts between 12 and 15 weeks. In the program, groups of six to eight people meet for three hours no less than three times a week. Clients focus on uncovering the underlying factors of their addiction and ways of treating them.

First Step utilizes cognitive behavioral therapy (CBT) and motivational enhancement therapeutic techniques. Programming also includes individual and group counseling.

The treatment center also uses creative experiential activities during group sessions. The activities could include River of Addiction, Trust Walk, art therapy exercises, small group interaction, and brainstorming. Programming also includes individual and group counseling.

The center's family program is free and educates family members, loved ones, and members of the community about addiction and recovery. First Step has one of the state’s 348 (73.7 percent) of family treatment programs.

The facility also offers an adolescent counseling program for teens seeking treatment for addictions. Treatment in this program includes assessments, individual and group counseling, family therapy, and drug screens. The program has flexible hours and offers late afternoon groups.

After clients finish the program, the next step is continuing care, which is designed to help individuals remain focused on recovery during life changes.


The treatment team includes licensed clinical social workers and licensed professional counselors. It is also one of the 207 (43.9 percent) facilities in the state that are accredited by CARF.

WHAT ALUMNI SAY has yet to receive any reviews from this facility’s former clients. However, the ten people who left Google Reviews for First Step Services at the time of this writing provided mixed reviews, and a 2.8 out of five star average rating.[1] Some alumni noted they had "horrible experiences" on Google, while others wrote "the customer service is incredible, the employees are patient and helpful!"


At the time of this writing, the sole loved one polled by to date gave First Step Services a one out of five star rating for its treatment effectiveness. The loved one wrote how the counselor at the NA meeting they attended was “inexperienced and she couldn’t answer some questions.” The reviewer indicated that the counselor “laughed at me” “behind my back” and that the program led him toward involvement in “heavier drugs.”


The one staff member polled by at the time of this writing provided a positive review of First Step Services, and gave five out of five stars for treatment effectiveness. "If I wasn't an employee, I absolutely would choose First Step Services, LLC.," they wrote. "We have had former patients return and become quite successful."

When first joining first steps NA meeting for our son the counsler was inexperienced and she couldn't answer some questions. It was apparent that she was trying to be more of their friend than counsler during sessions. During a session I mentioned something about my sons addiction. Behind my back she laughed at me and said what I said was stupid to the group. I was only repeating what out family counsler had said. My son became more involved with drugs and felt this was just an open door to expose him to getting him more acquainted with others that gave him heavier drugs. In a time when we are in need for help and vulnerable, I felt they just wanted our money.

Strong, patient/client-centered program, highest ethical standards. CARF accredited. Multiple locations in Raleigh, Durham, Garner & Cary NC. If I wasn't an employee, I absolutely would choose First Step Services, LLC. My employment at the centers would prevent me becoming a patient. Absolutely would recommend this facility because it is CARF Accredited, Fully DHHS NC Licensed. We have had former patients return and become quite successful. The decision would be based on the comprehensive assessment and consultation with the Clinical Director or the Medical Director. Patient centered, exceptional clinical program, strong relationships with community & referral base. Downfalls would be having to compete with low cost DWI programs that aren't required to be DHHS licensed or achieve any level of Accreditation. Our high standards carry a high financial cost. We network with all the majors that operate in our areas; we check benefits and provide authorizations, billing, overview of using insurance for behavioral health.
