ABOUT TRANSFORMATIONS WELLNESS CENTER
Located in Klamath Falls, Oregon, Transformations Wellness Center (TWC) offers residential treatment to adults struggling with substance use disorder. The facility is a private nonprofit, which make up about 53 percent of all treatment facilities in Oregon. The mission of the facility is to care for the whole person with an individualized approach and evidence-based therapies. Medically managed detox is not available on-site.
TREATMENT & ASSESSMENT
In order to be admitted, clients must first complete an assessment to determine their needs and screen for any co-occurring disorders. Treatment typically lasts 90 days and provides more than 42 hours of programming a week, including group and individual counseling. Clients are also encouraged to explore their own beliefs on spirituality throughout treatment.
The facility uses a variety of research-based modalities including motivational interviewing (MI), the Matrix Model, dialectical behavior therapy (DBT), cognitive behavior therapy (CBT), and training on various skills such as self-esteem and anger management.
The program also offers 12-step facilitation and relapse prevention training, and residents are expected to attend all 12-Step, church, and other group meetings.
The treatment team includes a physician's assistant who also serves as the medical director, several certified drug and alcohol counselors, resident assistants, and other administrative and support staff.

TWC is one of 37 (17%) non-hospital facilities in Oregon to offer residential treatment programs that last more that 30 days. There is currently no information provided by the facility regarding its living arrangements and related offerings,
WHAT ALUMNI SAY
On its Facebook page, which the rehab has the ability to manage, TWC earned an average rating of 4.4 out of five stars based on 14 reviews. Most former clients described the program as life-changing and praised the facility for its effective treatment. "Transformations wellness center saved my life, & now I'm able to help people so awesome and rewarding," Christina wrote in a representative review.
TWC also received a glowing five-star review on CiteHealth from an anonymous alum who noted: "The facility and staff were very accomidating from the minute I walked through the door... I learned about my abuse and what it does to my body and also about the mental illness I was experiencing."
WHAT STAFF SAY
At the time of this writing, Rehabs.com has received one review of TWC from a staff member named Jim. He gave the facility mostly positive feedback and awarded it five out of five stars for its overall quality of client treatment and care, as well as five out of five stars for its staff's level of experience.
According to its website, TWC takes Oregon Health Plan (Medicaid) and private insurance. The facility also offers fees on a sliding scale.
